Every Texas business starts here, regardless of type. Form your entity, get your federal EIN, and register for the taxes that apply to you. Most of these have no or minimal fees, of course we highly recommend you confirm the current amount with each agency before filing as believe it or not things change.
Healthcare is distinctive in Texas. For most of these business types the state does not issue a facility or clinic license. The practice is authorized by the individual professional license of the clinicians who work there. Home health agencies and non-PT-owned therapy clinics are the exceptions. The core requirement is that each dentist on the premises holds an active TSBDE Dental Licence — Texas issues no separate facility licence for a dental office. Practices offering sedation need an Anesthesia Permit, and any dentist prescribing controlled substances needs a DEA registration.

One of the few healthcare types Texas licenses at the facility level. You need a Home and Community Support Services Agency (HCSSA) Licence from HHSC, in the right category — licensed home health, licensed and certified home health, or hospice.

A PT or OT clinic is authorised by the individual licences of its therapists. A separate PT Facility Registration is required only where the facility is owned by non-PTs. PT-owned practices are exempt.
